Privacy notice

1. Data controller

Under Turkish Law No. 6698 on the Protection of Personal Data (“KVKK”), your personal data is processed by Orhan Demir (Dent4 Levent Ağız ve Diş Sağlığı) (the “Clinic”) as data controller, as explained below. In case of any discrepancy, the Turkish version of this notice prevails.

2. What data we process

• Identity and contact details you share through the appointment request form, by phone, WhatsApp or email: name, phone number and email address.
• Request details: your preferred day and time and your message.
• Health data (special category): the treatment area you may optionally select and any health information you choose to include in your message; X-rays and photos of your teeth you choose to send on WhatsApp. You do not need to describe your health condition in the form.
• Security data: access logs kept on the hosting server (IP address, date and time, browser information).
• Your cookie choice. See the cookie policy for details.

3. Why we process it

• To receive your appointment request, contact you and plan your appointment
• To answer your questions
• To keep the website secure and meet our legal obligations
• To remember your cookie choice

4. Legal grounds

• Identity, contact and request details: processing directly related to entering into a contract (KVKK Art. 5/2-c) and our legitimate interest (Art. 5/2-f).
• Health data you choose to share in the form: your explicit consent, given with the separate checkbox (Art. 6/3-a). During examination and treatment, health data is processed by our dentists, who are bound by professional secrecy, for diagnosis and treatment (Art. 6/3-e).
• Server access logs: our legal obligation under Law No. 5651 (Art. 5/2-ç) and legitimate interest (Art. 5/2-f).
• Cookie choice: necessary for the site to work (Art. 5/2-f). The Google Maps window is only loaded with your consent.

5. Who we may share it with

• Public authorities legally entitled to request it.
• Our hosting and email service providers, only as needed to provide the service.
• The clinic's email address (dent4levent@gmail.com) is provided by Google LLC; information you send by email may be processed on its servers abroad. Messages you send on WhatsApp are processed by Meta Platforms. These transfers abroad are made in line with KVKK Art. 9, with your explicit consent where required.
• If you allow the Google Maps window, Google may process data under its own privacy policy.

6. How we collect it

Through the appointment request form, phone, WhatsApp and email, and automatically while you use the site (server logs, the necessary cookie).

7. How long we keep it

For as long as needed for the purpose of processing and for the periods set by law; after that it is deleted, destroyed or anonymised. Server access logs are kept for the period required by Law No. 5651.

8. Your rights under KVKK Art. 11

You can ask whether your data is processed and request information about it; learn the purpose and whether it is used accordingly; know the third parties in Türkiye or abroad to whom it is transferred; ask for correction, deletion or destruction and for these to be notified to third parties; object to a result against you arising solely from automated analysis; and claim compensation for damage caused by unlawful processing.
